Is it white spots, or white fuzzy things growing out like cotton?

That most likely means its a fungus. Get a good fungus medicine like Myacin remove your filter cartidge, but leave your biowheel in and follow the package directions for your tank. After that I would suggest doing a 50% water change to clear the remaining medicine out of your tank.

Good Luck.

woah! never do a 50% water change! do a 20 and wait a few days and do a 20 and wait a few days and then do another 20% and make sure you keep topping the tank off when the water level drops. puffers are so cute but they are really prone to things like fungus and parasites. i don't know if you keep other fish with it or not but you shouldn't. my puffers killed everything once.
