Itchy area at the back of the rear end (right before the tail?) sounds like a reaction from flea bites to me. Is it a reddish patch of skin, perhaps with some dark specks? Is he an outdoor or indoor dog? If he's an indoor dog, have any members of your family had flea bites?

My dog is extremely sensitive to flea bites and has had this reaction a few times, as fleas are a problem year round in southern California. We finally started using Advantage rather than the harsh flea shampoos that dried out his skin and coat (which just made him more itchy), and this completely took care of the problem.

Have you found fleas on your dog with a flea comb or any evidence of dark red-brown/black gritty material? (Also be sure to check his bedding and other places where he spends a lot of time.) What is the special shampoo you mentioned? I'm assuming that's it's either a medicated or a flea shampoo, but even if the bath got rid of the fleas, the allergic reaction will continue to make him itchy. Also, it's very hard to get rid of fleas if they've already become a serious problem, so they are likely to bother him again if you just get rid of the fleas on the dog's body without treating the rest of his environment.

A quick trip to the vet would probably solve the mystery. Hot spots are common, so I would imagine your vet should be able to identify it quickly if it's caused by fleas.
