It it is a UTI, its probably a new one, I don't think a cat could go 4 months without showing more symptoms.

Yes, its premanent, although we don't yet know the extent. Right now its looking almost normal. But when the blood reabsorbs from the smaller hematoma, it could get worse looking again.
